MUI Reporting

A Major Unusual Incident (MUI) is the alleged, suspected, or actual occurrence of an incident when there is reason to believe that the event has occurred and the health or welfare of an individual with a developmental disability may be at risk. Click here for a list of MUI categories.

MUIs must be reported to the County Board in a timely manner so that the incident can be investigated, and preventative measures can be put in place to help ensure the health and safety of the individual involved.

If you need to report an MUI, please contact the individual’s Service and Support Administrator (SSA). Click here for a staff directory. If you do not know the individual’s SSA, you may also report an MUI by calling the County Board Office at (740)477.3353, Monday – Friday, 8:00AM – 4:30PM. If you are reporting an MUI afterhours, please call our on-call system at (740)497.2887. Once you have notified the County Board you will need to submit an Incident Report in writing to the SSA by 3:00PM on the following business day. Click here for a copy of a blank Incident Report.
